Tracheostomy Care Instructions...

Description: A tracheostomy tube is inserted to bypass an obstruction in the windpipe. (trachea) Tracheotomies can be temporary or permanent.

Procedure:

RNs and LPNs will perform and teach the following to patients and caregivers.

Supplies:

  • Tracheostomy cleaning kit
  • Peroxide
  • Saline

Cleaning Instructions:

  1. Wash hands.
  2. Open all supplies.
  3. Put on gloves.
  4. Clean around your trach with a Q-tip soaked in peroxide. Start on the side and wipe away from the tube.
  5. Repeat around the entire trach area turning the Q-tip with each swipe.
  6. Wipe beneath trach with a saline dampened 4x4 sponge.
  7. Dry with clean 4x4 sponge.
  8. Keep area under trach dry.
  9. Fold 4x4 sponge in half and place on each side of trach.
  10. 4x4 are changed as needed.

Cleaning Trach Tube:

  1. Wash hands.
  2. Open all supplies, pour peroxide in tray, and turn water on.
  3. Put on gloves.
  4. Remove inner cannula and soak in peroxide.
  5. Clean outside of tube with brush.
  6. Clean inside of tube by inserting brush in and out of tube several times.
  7. Rinse trach with water.
  8. Dry trach and insert into outer cannula.
  9. Shiley trachs are turned clockwise to secure.
  10. Discard supplies.

Changing Tracheostomy Ties:

  1. Wash hands.
  2. Open trach tie.
  3. Put on gloves.
  4. Hold trach securely.
  5. Remove old trach tie one side at a time.
  6. Insert new trach tie into slit on side of trach.
  7. Remove opposite side of old trach tie.
  8. Insert new trach tie into slit on side of trach.
  9. Adjust trach tie by pulling side of trach tie until you can insert your index finger between the skin and the trach collar.
  10. Attach the Velcro securely.
  11. Discard supplies.
